Materijal i metode. Diabetes mellitus affects 3% of Serbian population. Stroke is one of the major macrovascular complications of Diabetes mellitus. Aim. Determining what additional intervention, care and treatment of diabetic patients with stroke needs, when comparing patients with stroke who have other risk factors. Material and Methods. The research was conducted in the Specialized Hospital "Sveti Sava", at the Intensive Care B, during the period from the 1 November 2019 th to the 31 December Total number of patients was 40. Subjects were divided into two equal groups of 20 patients: the first group- the experimental group with diabetic patients with acute stroke, and the second group- the control, with patients with acute stroke who have other risk factors. The study was based on general standards of nursing interventions, and advanced standards in the care and treatment of diabetic patients with stroke were used. Results. According to "Nursing activities score of basic nursing interventions: continuous monitoring was present in 13 (65%) patients in the experimental group whereas the control group that had 5 (25%) patients. Monitoring of vital parameters and the number of nursing interventions was much higher in the experimental group. Conclusions. Using the common standards of nursing interventions in the care and treatment of diabetic patients with acute stroke is not enough. Their demands are highlighted and they require longer time and the increase number of nurses for the implementation of the plan of care and treatment for these patients. Patients with stroke are complex and prone to numerous complications and serious approach to each patient individually is needed. 7 INTRODUCTION Diabetes mellitus is a common metabolic disorder with more than 1.7% of the world population suffering from it (according to some studies the number goes to 3.5%). Diabetes mellitus affects 3% of Serbian population, with a tendency to increase [1-7]. Stroke is one of the major macrovascular complications of diabetes mellitus, and when comparing the incidence of mortality it is the third cause of death in the world (right after cardiovascular diseases and cancer). 60% of patients with stroke have a hyperglycemia as well; even though diabetes wasn't mentioned in the history that has been previously given. It is believed that genetic predisposition together with participation of the environmental factors is involved in its ethiology [2, 8-10]. There are 2 types of this disease: type 1 Diabetes mellitus and type 2 Diabetes mellitus. Type 1 Diabetes usually develops rapidly since the day of birth, so the symptoms are more pronounced. Type 2 Diabetes for a long time doesn't cause any unusual problems as it is developed very gradually during a long period of time, and with concealed clinical signs. Sometimes the type 2 diabetes is detected only when the complications of diabetes occur [3, 5-8]. The health care of these patients is extremely complex and requires a good assessment of the patient's condition and a good care plan. AIM The aim of this research is to determine what additional interventions, care and treatment measures of diabetic patients with stroke should a nurse do, compared with those who have other risk factors. The tasks of this research are: - To examine in what measure diabetes is present as a risk factor in patients with stroke hospitalized in the Intensive care unit B in the Sveti Sava hospital - To analyze other risk factor for stroke that were present in hospitalized patients, and to examine to what extent were they present in the total number of hospitalized patients. - To determine type and the number of standardized nursing activities in diabetic patients with stroke in comparison to the group of patients with stroke that have other risk factors. - To assess the state of consciousness of the diabetic patients with stroke - To assess the risk of damaging the integrity of the skin in diabetic patients with stroke - To examine the intensity of pain which is caused by the damages to microcirculation in diabetic patients with stroke MATERIAL AND METHODS The research was conducted in the Specialized Hospital for prevention and treatment of cerebrovascular diseases Sveti Sava in the Intensive Care unit B, in the period of to year. Total number of patients was 40. Methods A. Descriptive method B. Indirect observational method - documentation analysis Direct observational method: - current (observation after some periods of time) - interview Techniques and instruments of the research: scaling techniques, interview and analysis techniques. Instruments used in the research are: I. Scales: - Nursing Gold Standards in Intensive Care Units - Nursing Activities Score (NAS), used for scoring nursing activities - Glasgow Coma Score (GCS), used for the assessment of state of consciousness - Norton scale, used for the assessment of the risk for decubitus ulcer - Numerical scale for the pain assessment - Admission nursing list as an interview questionnaire II. Documentation: - documentation of the process of medical care and the history of the disease RESULTS During the period of year to year, at the Intensive care unit B, there were 271 hospitalized patients with acute stroke. There were more SESTRINSKA VIZIJA. broj 8. jun

8 men than women in both groups of respondents. The prevalence of intensive care patients was approximately the same in both study groups (Fig.1, Fig.2). The major risk factors for stroke were analyzed. The major risk factor that was singled out was the one that was previously diagnosed, and for whom the treatment measures have been taken. If the risk factor hasn t been present in patients, it was noted as unknown risk factor (Table 1). All the patients of the experimental group in their admission diagnosis had diabetes as a medical diagnosis, as well (Table 2 and 3). According to the Glasgow Coma Score, the opening of eyes was monitored because it is the one of the best verbal and motor responses. In the group of diabetic patients there was 4 patients (20%) with the Glasgow score below 7 (which signifies coma), while there were no such patients in the control group. According to the Norton scale, where the physical condition, mental status, movement, bed mobility and incontinence of urine and stool were monitored, in the group of diabetic patients there were 12 patients with high risk (60%). According to the Nursing Activities Score (NAS), basic nursing interventions such as continuous monitoring (presence of the nurse with the patient with activities that last more than 2 hours per shift) was present in 13 patients (65%) in the experimental group, and in 5 patients (25%) in the control group. After one hour, monitoring was requested for 4 patients (20%) in both the experimental and the control group of patients. Monitoring of vital parameters was requested after 3 hours for the rest of the patients of both groups.the demand for laboratory monitoring was significantly higher in the experimental group of patients (Fig. 3). The most frequent oscillations that happened in the both groups of patients were those in arterial pressure and in the number and the quality of respiration. Interestingly, respiratory and cardiovascular support were more prevalent in the control group, while renal and metabolic support were more prevalent in the experimental group (Fig. 4). Natural and enteral nutrition were present in both groups of patients, although most patients in the control group required enteral nutrition (Fig.5). The subjective feeling of pain was emphasized in the patients in the experimental group (Fig. 6). Hygienic procedures, together with mobilization and positioning procedures were carried out for more than 2 hours during the shift in 16 patients (80%) of the first group of diabetic patients, and more than 4 hours in 4 patients (20%). Whereas in the control group of patients, 12 (40%) of them were treated for more than 2 hours and the other 8 (40%) for less than 2 hours. DISCUSSION The results confirmed the fact that much more time should be given to the hygienic care of diabetic patients and it can be concluded that the hygienic care has a huge importance in the care of immobile patients with stroke, especially the diabetic patients with stroke. The impact that hygienic care has in the quality of recovery is invaluable, and it can be classified in the same category as timely treatment of stroke [5-8]. Working with the patient's family has been only informative so far, and slightly educational. Given that all patients belonged to the intensive care unit according to the seriousness of the condition, time that they had after visiting hours was spent on the family related explanations about the patient's condition, as well as on the advice on how they can help their loved ones. The family is trained on how to feed the patients, how to establish communication with the patient and how to use stimulutive effects for better recovery [9, 20-22]. The recommended standard- Nursing Gold Standards in Intensive Care Unit showed much more critical results of patients at risk, around 90% in the group of diabetic patients, whereas 50% of such patients from the control group of patients who had other risk factors. This way we were able to identify a shocking data- there was a lack of nurses who had to treat those patients. 4 nurses have been engaged for the care of patients in the first group, whereas 3 nurses were engaged for the care of patients in the second group during the first shift, while the other two shifts had even fewer nurses. It is a large deviation from the standard which consists of 8 nurses per shift [10-14]. Nurses have exceptional responsibility when implementing nurses interventions and also for the timing of the administration of insulin and other medications for diabetic patients on the one hand, and on the other- to prevent aspirations of food into the airway of the patients that have trouble swallowing [10]. Patient s nutrition demanded diabetic diets for every patient in the group of diabetic patients; whereas in 6 SESTRINSKA VIZIJA. broj 8. jun 2021.

9 only 6 people with rises of blood glucose levels from the group of patients with stroke that have other risk factors. This data supports the fact that over 60% of patients with stroke have hyperglycemia, even though they are not diabetic. Researches of the nurses from the Republic of Croatia (Croatian Journal of Public Health) showed similar results in separate studies of patients with diabetes and patients with stroke as well as other studies [11-18]. The number of all the nursing interventions was enormously increased in the group of diabetic patients.whereas the cardiac and respiratory support were slightly more increased in the control group of patients. Metabolic support was increased in the experimental group of patients, where there was frequent need for the correction of acid-base status. Renal support for monitoring of renal diuresis was also increased in the group of diabetic patients (65%). According to the information in literature, pain that is associated with diabetic neuropathy is sometimes missing, but it was not the case in this study, although there was no recorded data on diabetic neuropathy [10]. Nurse s diagnosis, outcomes and nursing interventions in this study Patient's problems: - Imobility - Altered state of consciousness - Vertigo - Sickness, disgust - Vomiting - Altered visual perception (diplopia) - Discomfort caused by the inability to hold urine and stool - Pain in the stomach area- inability to urinate - Swallowing difficulty - Feeling of suffocation - Difficult communication Nurse s diagnoses: - Potential damage to the integrity of the skin caused by immobility, which is usually manifested with transient redness - Altered state of consciousness - The risk of falling due to the dizziness and changes in the visual perception - Subjective discomfort associated with the urge to vomit, which is manifested with nausea and disgust - High risk of aspiration, due to the dysphagia and vomiting - Altered elimination of urine and stool which is connected with the inability to control sphincter, manifested as incontinence of urine and stool, or as pain in the stomach and urine retention - Altered food intake related to primary illness, manifested as swallowing difficulty - Inefficient drainage of secretions from the respiratory tract, manifested as suffocation - Deterioration of communication Outcomes: - The integrity of the skin won't be damaged - The improvement of the state of mind will occur - The symptoms of vertigo, nausea and disgust will be reduced. - Will be no aspiration of vomit - The improvement in visual perception will occur - The problem of urine and stool retention will be removed, abdominal pain will disappear - The airway clearance will be sustained - A better communication will be developed Standardized interventions of the nurses and care plane Patients with stroke, whose body part have been paralyzed, become immobile, and the starting risk assesment for decubitus ulcer is of great importance so that preventive measures could be taken at the right time. With nurses history, the Norton scale is widely used for the risk assessment of decubitus ulcer. At the same time, the Morse scale is used for the risk assessment of falling. Plan of care, which is individual for every patient after the assessment, is the foundation of interventions [4-8]. Undeniably, it is essential that these patients receive proper bed with anti decubitus mattresses or other improvised aids, such as protected fences which improve patient's safety. In order to prevent other immobility complications- thrombophlebitis, hypostatic pneumonia, urinary tract infections and others, corrective positions, hygienic treatment, full body massage (especially on predilection places) with nourishing oils and the changes of patient's position every 2 hours are of vital importance [12-15]. SESTRINSKA VIZIJA. broj 8. jun

10 Due to the good perfusion, the patient's head is slightly elevated at 30 to 45 degrees. Breathing excersises and percussion are the standard procedures for the prevention of hypostatic pneumonia [10-12]. Any participation of the patient in self-care activities with his remaining abilities, and the inclusion of the family favourably affect the recovery process. In order to improve vertigo problem, it is important to explain to the patient that it is a passing phenomenon, with the emphasis on the patients not making any sudden movements. If the visual perception is altered, the patient should be assisted by alternative closing of an eye [11, 12-18]. Therapeutic support, with the physician instructions, will certainly relieve sickness, nausea and vomiting, although monitoring of the patients is essential in order to prevent possible aspiration. Permanent monitoring in the acute phase of the disease is essential, and it includes 24-hour supervision. As far as incontinence of urine and stool is concerned, the application of adult diapers is preferred, unless the monitoring of urination is required. It greatly reduces the inconvenience that may occur with the patient. Bladder catheterization always carries the risk of urinary tract infections, so careful preventive measures, placement and maintenance of urinary catheters should be considered [11-14, 19-22]. The problem with swallowing must be carefully assessed in order for the best solution to be selected. The nasogastric tube in the absence of the swallowing reflex is a solution, and its good maintenance is necessary to prevent infections. Parenteral nutrition is not an option, because the latest findings speak in favor of the immune barrier in the gut, so the enteral nutrition is a priority. According to the assessment of the situation, patients monitoring of vital parameters are of outmost importance to the recovery, and nurses must be well trained to recognize such abnormalities, so it could be responded in time [13-19]. A common problem of patients is communication due to the inability to speak or understand speech, so patience when communicating with those patients should be a common trait of the nurses. The involvement of family is also very important, as well as resorting to modern stimulation methods of treating patients with stroke [12-15].. It is necessary to include other members of the team such as: a speech therapist, psychologist, dietician, social worker; although, the mandatory team members are: a nurse, neurologist, internal medicine specialist and physical therapist [12-22]. CONCLUSION 1. According to the obtained results, it can be concluded that all the goals and objectives of this study have been realised. 2. Interventions of the nurses in the care and treatment of diabetic patients with stroke are extremely complex and specific. According to the assessment of the situation, when comparing nursing interventions in the care and treatment of diabetic patients with stroke and patients with stroke who have other risk factors, it was found that there should be additional nurses activities in the care of those patients. 3. All standardized nursing interventions in the care and treatment of diabetic patients should be emphasised and require a longer time and greater involvement of nurses for the implementation of that treatment plan. However, as the patients with stroke are complex and prone to numerous complications, they are in need for serious and individual approach regardless of risk factors. 4. The results have shown that the scales that were used in this study are applicable in everyday practice, and that they are useful in the evaluation of the nurses work. They give results of current situation on the number and type of nursing interventions, the workload of nurses in health-care team, and about the number of nurses that differs from the standardized number, according to the severity of patient's condition. INPLICATIONS FOR NURSING PRACTISE The scales that were used in this study are applicable in everyday practice. Infekcije uzrokovane multirezistentnim (MDR) bakterijama predstavljaju globalan problem, ograničavajući izbor antimikrobne terapije. Infections caused by multidrug-resistant (MDR) bacteria represent a global problem, limiting the choice of antimicrobial therapy. Objective: To determine the rate of infections in ICUs, as well as the type of MDR bacteria associated with such infections. Material and methods: The study included strains of MDR bacteria isolated from laboratory samples of hospitalized patients in surgical ICUs in a one-year period. All strains of MDR bacteria were isolated and identified by standard microbiological methods for aerobic bacteria according to the recommendations and guidelines given by the Clinical and Laboratory Standards Institute (CLSI). Results: Infections caused by MDR bacterial isolates were confirmed in 199 (19.17%) patients in ICUs. The frequency of Gram-negative isolates was significantly higher than Gram-positive (88.35% and 11.65%, p <0.001). The most common infections in ICUs caused by MDR bacteria were respiratory tract infections (49%), followed by soft tissue and skin infections. The CDI incidence density 1.13 cases/10,000 patient-days. Conclusion: A high frequency of infections caused by MDR bacteria was found with patients in ICUs, and respiratory tract infections dominated. Information on the types of infections and pathogens can help develop policies for prevention, diagnosis, treatment, and resource allocation. 16 UVOD Povećana učestalost multirezistentnih bakterija (engl. multidrug resistant - MDR) poslednjih nekoliko godina predstavlja problem u javnom zdravstvu. Pridržavanje mera predostrožnosti i agresivnije čišćenje okoline mogu smanjiti prenos ovih bakterija [12,13]. Iako antimikrobna rezistencija raste u JIL širom sveta, postoje i značajne razlike u specifičnim obrascima i stopama MDR-a širom zemalja i geografskih regija [14].

Infekcije stečene u JIL povećavaju troškove hospitalizacije pacijenata, a takođe čine gotovo polovinu ukupnih bolničkih troškova.

[30]. Rezultati istraživanja Ziółkowski i sar. S druge strane, metaanaliza Baur i sar. LITERATURA 1. Statistically significant difference has not been established between four quality of life domains (Physical health, Conclusion: Psychological, Social relationships, Environment) and sample stratifications: gender, age, studies, work status and marriage status. Quality of life can influence daily functioning of individuals in various life domains. Key words: Quality of life, Perception, Students, Higher education, Health care SESTRINSKA VIZIJA. broj 8. jun

Postoji širok opseg definicija i pristupa kvalitetu života. Svetska zdravstvena organizacija definiše kvalitet života kao ličnu percepciju sopstvenog načina života u kontekstu kulture, sistema vrednosti, težnji, izgleda za budućnost, standarda i interesa [3]. Možemo primetiti da je glavni kriterijum istraživača subjektivnog pristupa kvalitetu života lično viđenje sopstvenog života. Pored subjektivnog aspekta, postoji i objektivni aspekt kvaliteta života koji podrazumevaju objektivne karakteristike osoba i uslova u kojima one žive. Validnost i pouzdanost WHO- QOL-BREF upitnika na srpskom jeziku potvrđena 20 SESTRINSKA VIZIJA. broj 8. jun 2021.

Statistička obrada i analiza urađena je u statističkom paketu SPSS v24 (Statistical Pasckage for the Social Sciences) for Windows. U cilju ispitivanja pouzdanosti mernog instrumenta, koristili smo Krombah alfa koeficijent. Sve skale imaju dobru pouzdanost, tj. pouzdanost iznad 0,70 (Tabela 2.). Skala kvaliteta života meri pet dimenzija: generalni kvalitet života, fizičko zdravlje, psihičko stanje, socijalno stanje i okruženje. Veći kvalitet života imaju student iz Loznice (Me=9,00) i Kruševca (Me=9,00) u odnosu na studente iz Beograda (Me=8,00), p<0,05. Psihičko zdravlje ispitanika prve godine je bolje (Me=26,00) u odnosu na one koji su druga godina (Me=24,00), p<0,05. Veći kvalitet života u oblasti psihičkog zdravlja imaju studenti iz Loznice (Me=26,00) i Kruševca (Me=26,00) u odnosu na studente iz Beograda (Me=24,00), p<0,05. 26 shvatanje kvaliteta života i stoga osobu treba posmatrati u socijalnom kontekstu, naglašavajući važnost psihosocijalne perspektive.

ABSTRACT Introduction Nutrition has a great impact on the health potential of young people. It is very important to analyze aspects of adolescent nutrition in a timely manner, in order to identify potential health risks. Objectives The aim of the study is to examine adolescents' eating habits and attitudes toward obesity. Methods 370 students from the Valjevo Medical School, Western Serbia, participated in the study. The survey was designed as a cross-sectional study, using the questionnaire whose first part was related to demographic characteristics of participants, the second part of the instrument examined eating habits, while the third part dealt with obesity, using the AFA obesity scale. Scale performance was tested by Cronbach's alpha test (α =.78). Results The results showed that the majority of the subjects were eating properly and without skipping meals. It was worrying that the consumption of sweets and sodas was high. Females stated in large numbers that they disliked obese people. Most of the differences between healthy and unhealthy habits come from the living environment and parental level of education. Conclusions The transition years have brought some irregular eating habits in the adolescent population, more education is needed to maintain a healthy life. Key words: Adolescence, healthy eating, adolescents' attitudes about obesity. 26 SESTRINSKA VIZIJA. broj 8. jun 2021.

29 INTRODUCTION South-East Europe, particularly West Balcan is going through a huge transition. Significant political and social changes have occurred during the last decades and rapid socio-cultural changes put adolescents at risk for developing various health problems. Adolescence is a complicated developmental period, with both psychological and physiological changes. Also, this is a vulnerable stage of life in terms of nutrition, with a higher prevalence of food-related disorders owing to lifestyle and eating habits. The nutrient needs of adolescents are higher than those of children and adults. Also, parenteral control regarding eating habits reduces in this period of life. In addition, the composition of the diet is not adequate, i.e. too much protein, fats, sugar are being used, while there is an insufficient intake of vegetable, fruit and milk products in adolescents diet. Skipping breakfast and taking snacks are highly prevalent among adolescents. Consuming unhealthy foods throughout the day is common. Owing to their school duties, adolescents are more likely to eat away from home on a daily basis. Food-related disorders, including eating disorders, under nutrition, overweight and obesity are very common in this population. Although contemporary media propagate thinness and slenderness, obesity is a serious health concern not only for adults but for children and adolescents as well. The rising prevalence of obesity and metabolic syndrome among adolescents put the whole population at great risk of developing non-communicable diseases, which have been of growing concern in many counties in transition. According to the World Health Organization, the major causes that lead to obesity during childhood or adolescence are increased energy intake, decreased physical activity, sedentary lifestyle, changing modes of transportation, increasing urbanization and other societal reasons [1]. Due to media promoting fit and thin appearance, there are a lot of antifat attitudes and stereotypes toward obese people among children, adolescents and adults, by attributing to them more negative and fewer positive traits than to normal-weight people. Studies examining eating habits of adolescents, as well as antifat attitudes among them, are very limited in Serbia [1-3]. The aim of the study was to examine attitudes toward overweight and obesity among medical school students, to determine differences in attitudes toward overweight and obesity according to different levels of socio-demographic data (gender, parenteral education and living area), as well as to investigate eating habits among medical school students. METHODS The research was conducted in the form of a cross-sectional study. In total,370 students from the Medical School Dr Misa Pantic in Valjevo, Serbia participated in the study. The sample consisted of students from all grades and from the following educational profiles: Nurse, Pediatric Nurse, Physiotherapy Technician, Laboratory Technician and Pharmaceutical Technician. The criterion for inclusion in the research was provided written consent of the parents and personal consent of the student to participate in the study. The exclusion criteria were the lack of written consent to participate in the research, as well as the lack of readiness of students to answer all the questions in the questionnaire. The research instrument was a questionnaire, specially designed for the purposes of this research. The time required to complete the questionnaire was 45 minutes. The first part of the questionnaire contained questions related to the demographic characteristics of the respondents, namely gender, age and family aspects. The second part of the research instrument examined student s eating habits. The third part of the questionnaire examined student s attitudes towards obesity. In this part of the study, the Antifat Attitudes Questionnaire (AFA) and Obesity Attitude Scale were used. The efficacy of the scale has been repeatedly demonstrated in the available literature and tested by the Cronbach's alpha test (α =.78). Antifat Attitudes Questionnaire consisted of 14 claims, with a modified three-level Likert scale (1 for dissent, 2 neutral, 3 for agree). The research was approved by the competent authorities of the Medical School Dr Misa Pantic in Valjevo. Informed consent was obtained from all individual respondents included in the study. The survey was conducted from September to October of The research sites were the school premises and classrooms. The sample of students was intentional. Prior to the start of the study, students and parents were provided with the necessary information regarding the study itself, both verbally and in writing. Students were given instructions regarding how to complete the questionnaire correctly. Questionnaire distribution and data collection were conducted by the researchers personally. Weight and height were measured according to a standardized SESTRINSKA VIZIJA. broj 8. jun

30 protocol. Body weight (kg) and height (cm) were measured with an electronic scale and stadiometer. Body mass index (BMI) (kg/m2) was calculated as body weight (kg) divided by the height squared (m2). Data processing involves descriptive statistics methods. The results are presented graphically and in tabular form. RESULTS The largest number of respondents was female, male respondents made up the sample of 42%. Respondents indicated that they lived mostly in urban areas (63%), less in a rural environment (37%). The majority of respondents lived with both parents (89%); the majority of parents have only completed secondary school (Supplementary Table 1). (10%) for the lunch. Among the respondents, the majority of students receive information on healthy nutrition via the Internet (36%), a total of 33% in discussions with parents, with professors 17% and only 14% from health professionals. The largest number of students is involved in sports 81%, only 19% was not engaged in the sports activity. A tabular view of adolescent eating habits is given in Table 2. The majority of the students had a normal body mass index (71%), those with overweight and obesity were represented by 23%, and underweight by only 6%. When it comes to the attitudes of the respondents regarding obesity, in most cases female students had a more negative attitude. Male students expressed fewer negative attitudes toward obesity and obese people. In a minority of cases, discriminatory behavior Table 1. Most students (79%) stated that their meals were diverse. In the study s sample, the largest number of students consumed breakfast between 6 and 9 AM (67%). The largest number of students had dinner between 6 PM and 8 PM (74%). Among them, the majority consumed sweets and snacks several times during the day (44%), once a day 23%, 2-3 times a week 19%, never 14%. Among the respondents, the majority were students who consumed carbonated, energy and alcoholic beverages (82%). Most of the respondents consumed food from the bakery (69%), minority brought food from home and support for stereotypes and prejudices regarding the obese population were manifested. The results are moderately negative when it comes to the self-control and self-esteem of obese people. Fear of gaining weight is more pronounced in female subjects in the observed sample (Supplementary Table 3). DISCUSSION Overweight is seen as a problem not only in wealthy countries, but in low and middle-income countries, and it is rapidly growing [4]. The proportion of overweight children aged between 5-19 rose from 10,3% in 2000 to 18,4 % in 2016 [5]. This increase was driven by rising intake of calories, a shift from traditional to modern diets, urbanization and decreasing levels of physical activity. 33 lence of overweight and obesity has increased greatly worldwide, and in Serbia, according to the Ministry of Health, there were 20.2% overweight and 8.9% obese adolescents in 2013,which is a significant increase compared to Among our participants, 23% were overweight and obese, and these results are better than national data. Considering that it is known that obese people in adolescence most often remain obese throughout their lives, we believe that it is necessary to increase health education work in order to prevent obesity and other eating disorders. Having in mind that about one third of our participant had obese relatives, the explanation of our results may be in eating habits and engaging in the sports activity. Our students were asked to report if they are engaged in vigorous-intensity physical activity outside school hours, and 81% stated yes, which is in accordance with west Serbian data. Boys from the urban environment, with highly educated parents, are more engaged in the sports activity. Recent evidence suggests that having a high level of sedentary behavior has physiological and health implication [6].Our participants generally (77%) stated sedentary behavior after school hours, but only 2% of them spent more than 3-4 hours watching TV and playing video games. This is very optimistic data, comparing with results from other countries, owing to the fact that our participants are medical students, aware of the potential benefits of regular sports activity. The dietary patterns of participants show great variability. Almost half of them had three meals per day, and two thirds of them 3-4. Students from the urban environment, with highly educated parents, stated they eat diverse and healthy meals. Only 3% of them had less than three meals per day, so under nutrition is not the issue in west Serbia. Most of our participants (81%) stated their meals were diverse. Previous studies have found that binge eating is the second prevalent eating disorder, particularly in youth who are overweight or obese [7,8]. Only 12% of our responders stated they never had binge eating, while others reported excessive eating every week, daily, or seldom. Binge eating has been linked to emotional stress, anxiety, mood disorders and low self-esteem, all frequently seen among adolescents [9]. In our study, participants were likely to choose carbonated sweet drinks regularly, in 64%, especially in boys from the urban environment and from less-educated parents. Those beverages are very popular among adolescents worldwide, but are very dangerous due to their high energy density and potential impact on osteoporosis [10]. Regarding snacks, there are very limited targeted guidances for adolescents. Snacking was very common among our participant, and most of them (88%) consumed sweets, slightly less in girls. Meal skipping rates may be highest during the adolescently period, with breakfast and dinner being the most frequently skipped meal (14 88% and 4 57%, respectively) [11]. Skipping breakfast was uncommon in our study. Only 8% of participants skipped breakfast, 43% had breakfast early in the morning, and 49% later. Skipping dinner was also unusual (only 6%) and only 7% of our participants had late dinner. This is a satisfying result because it is well known that skipping breakfast is common among adolescents, especially in girls. Late dinner can be potentially harmful considering obesity, as well as sleeping patterns. A study that explored dietary habits in Poland students showed that 25% of students did not eat breakfast, 45,6% snacked in between main meals and 25% ate just before bedtime [12]. There are some commonalities in food preparation; in west Serbia, cooked meals are very common, and this could be the explanation why they were the most frequent choice in our survey. Meals cooked at home have a higher amount of essential nutrients and a lower amount of unhealthy ingredients [1]. Only 13% of our participants eat fried, and none of them deep-fried food. However, the contemporary dietary habits include some new food that today adolescents are very fond of, such as pizza, sandwiches and sweets. US adolescents over consumed fast food, which led to a high prevalence of obesity among them [13,14]. During lunch time, the majority of our participants, usually from less-educated parents, consumed unhealthy food from the bakery, which is typical for West Balcan. 44% of the participants reported consumption of sweets regularly, on daily basis, 23% with a frequency of 2-3 times per week, and only 3% consumed no sweets. Sweets were heavily consumed in our study, comparing with another study [15]. We see that as a problem, because, sweets are tasteful, but with high energy density, and a lot of negative health outcomes. On the other hand, restricting access to tasteful foods, whether self-imposed or by parental control, may have potentially negative consequences [16]. Having a fear of getting fat is an important question, because adolescents may be mainly sensitive to their weight and body im- SESTRINSKA VIZIJA. broj 8. jun

34 aging, which may lead to risky behaviors [13]. Only 22% of our participants stated the fear of getting fat, but 73% never had a restrictive diet, compared to 40,8% of participants in a study of Napolitano et al. [17]. Different results were obtained in studies from France, Spain, and Portugal [18-20]. Among adolescents who were on a restrictive diet in our study, the waist majority conducted diet recommended by the Internet or friends instructions. School, parents and the Internet equally influenced the knowledge about healthy nutrition among our participants. Considering that almost all of our participants had access to the Internet and television, it is important that advertisements on those media contain professional advice regarding healthy nutrition. Questions regarding the need for additional education about nutrition revealed that 85% of participants felt like they needed more information about healthy nutrition; thus, again, media, school and parents are important players in this part of their kid's education. Nowadays societies highly stigmatize obesity, leading to negative attitudes and behavior toward obese individuals [21,22]. Moreover, overweight and obese adolescents face an increased likelihood of being socially marginalized by their peers. They are less popular, and additionally, there are beliefs that weight is controllable, and that fat people have their own responsibility about weight. In our study, participants declared their fear of being fat and put additional weight on, but they did not blame fat people for being fat, and did not think that obesity is attributable to a lack of willpower and personal responsibility. A gender difference was found for explicit antifat attitudes, with females reporting more dislike of fat people, which is different from one study [23] but in accordance with others [21,22]. It is obvious from our results that females displayed more dislike attitudes towards fat people than boys did. While many adolescent girls desire to be thinner, boys desire to be more corpulent, and this may be the explanation for the results of our study. No one in our study stated dislike of fat people, or any doubt about their intelligence and reliability. Generally, they revealed tolerance, no prejudice and a non judgmental attitude. This is important because weight discrimination is prevalent, particularly in American society and among women [24]. But, as it was shown in a study from O Brien, commonly used measures of antifat attitudes do not appear to be adequate predictors of antifat discrimination, so we urge for more research and antifat discrimination measures [23]. CONCLUSION A healthy diet and the right attitudes of young people regarding this topic are important aspects of the health of the adolescent population. Numerous cultural, economic and social factors affect health. The health potential related to nutrition is influenced by the health literacy and quality of life of adolescents. Years of transition brought some improper eating habits among adolescents in west Serbia, so more education are needed to sustain a healthy lifestyle. ETHICS DECLARATIONS Conflict of interest The authors declare that they have no conflict of interest, the research respects ethical norms and principles. 32 SESTRINSKA VIZIJA. broj 8. jun 2021.

Istočnoevropske onkološke medicinske sestre/tehničari se mogu usavršavati uz pomoć EONS-a (Evropskog onkološkog društva medicinskih sestara/tehničara) kao krovne organizacije koja je lider u svim područjima zdravstvene nege onkoloških bolesnika, istraživanja, prakse, kontinuirane edukacije, komunikacije i pregovaranja radi boljeg prepoznavanja onkološke zdravstvene nege širom Evrope. Ključne reči: edukacija, sestrinstvo, onkologija, zdravstvena nega ABSTRACT European Union (EU) has defined the process and norms of nursing education. As a source of data, Directive 2005/36/EC and Directive 2013/55/EU was used together with Croatian laws and regulations which are related to nursing education. Nursing education in Croatia is carried out through high school education and also at the university level. The situation is similar or the same in many other Eastern European countries. Experience in nursing education in different European countries can be useful in improving nursing education in Croatia. Cancer nursing specialization in the Republic of Croatia, as in many Eastern European countries, exists only on paper, unlike in Western Europe, the USA or Australia. Eastern European cancer nurses can be trained with the assistance of EONS (European Oncology Nursing Society) which is an umbrella organisation providing leadership in all areas of cancer nursing, research, practice, continuing education, communications and advocacy for better recognition of cancer nursing across Europe. Key words: education, nursing, cancer nurses, oncology 34 SESTRINSKA VIZIJA. broj 8. jun 2021.

37 INTRODUCTION Nursing education is a permanent process that serves for personal and professional development [1]. Nursing education in Croatia has a long tradition, and since the beginnings of education in 1921 till these days it has gone through many changes. Today nursing education is based on the World health organization s Strategy for nursing and midwifery education and recommendations of European Council Directive 2005/36 on the recognition of professional qualifications. Education is regulated by The Act on Scientific Activity and Higher Education and principles of the Bologna process is applied [2]. Unfortunately, these strategies still do not regulate the issue of specializations in the nursing profession. DISCUSSION The present status of nursing education in Croatia is a controversial matter. Croatia is the only country that trains general care nurses at the level of five-year secondary vocational education. It is also the only form of secondary education in Croatia that lasts five years with a program of 4600 hours in three years of secondary vocational school for nurses and the previous two years of general education. In the Republic of Croatia, only secondary schools for nurses have a 10-year basic education: 2 years of general education + 3 years of vocational education [3]. Significant changes occurred in 2005 with changes of The Act on Scientific Activity and Higher Education and application of principles of the Bologna process which enabled education in nursing in two cycles. By finishing the first cycle of nursing study students gather 180 European Credit Transfer and Accumulation System (ECTS) credits and become a bachelor of nursing. The education at first cycle with a uniform curriculum is provided in four educational institutions in Croatia, whereby efforts are made to establish more institutions for nursing education. Every year about 300 students enroll in nursing studies. After finishing the first cycle, the nurse can continue studying nursing on master studies/nursing specialisations and gather another 120 ECTS credits. There are two nursing specialisations in Croatia at the moment: Management in Nursing and Public Health [3]. Upon completion of the university graduate study, participants acquire 120 ECTS credits and the title of master of nursing, which allows them to continue their education in one of the doctoral studies. For the time being, the enrollment of the doctoral study is enabled at the medical faculties or other faculties within the University [3]. Therefore in Croatia, there is a five-year vocational education as a basic nurse education, then undergraduate professional and university nursing studies, after that a graduate professional and university studies in nursing, and finally doctoral studies in biomedical sciences, thus reaching a total number of 14 different terminologies for nurses. On the one hand, forms of education are provided at different levels (undergraduate, graduate, professional and university nursing studies), but on the other hand, the health system does not recognize and valorize nurses as experts. Regardless of education, the largest number of highly educated nurses receive a bachelor s salary. There are exceptions that depend on the workplace and the institution of employment (management position, nurse educators, etc). Considering specializations in nursing in the Republic of Croatia, the situation is the same or similar as in most other Eastern European countries. Cancer nursing specialization, like other nursing specialization, exists only on paper, unlike in Western Europe, the USA or Australia. Historically, nurses have had a special role in the care of patients with cancer. However, the expanded research and treatment program against cancer that has occurred during the past quarter-century has been a catalyst for the development of oncology nursing as a separate specialization [4]. The recognition of cancer as a major national health problem was a key for formally establishing the specialization of oncology nursing in the USA, Australia and most Western European countries. This increased attention to cancer coincided with and complemented a major new emphasis in the nursing profession toward expanded roles in comprehensive patient care. Many oncology nurses first worked as both nurses and data managers for cancer research studies. As oncology called for increasingly more complex therapy, the collaborative relationship between nurse and physician became the best way to provide uniquely comprehensive patient care [5,8]. The roles of the oncology nurses vary from the intensive care focus of bone marrow transplantation to the community focus of cancer screening, detection, and prevention [6,8]. As the healthcare delivery system changes and new scientific discoveries are integrated into cancer care, the role of the oncology nurse will continue to evolve. Oncology nurses currently work in a variety of roles and settings that were unheard of SESTRINSKA VIZIJA. broj 8. jun

38 10 years ago but are nowadays increasingly common. In the ambulatory setting, oncology nurses function in nurse-run clinics that provide services such as long-term follow-up care to patients with cancer, prescreening prior to chemotherapy administration, the management of fatigue, or general symptom management. As the field of cancer genetics has developed, so have the roles for advanced practice nurses in the provision of cancer genetic counselling and risk assessment [7,8]. Considering the above examples, it seems that nursing specializations in Croatia are still undeveloped comparing to the rest of the world. However, as a source of further development of nursing specializations, EONS (European Oncology Nursing Society) can be helpful. What is EONS? Why is so important for cancer nursing and cancer nurses in Eastern Europe? European Oncology Nursing Society (EONS) is an umbrella organisation providing leadership in all areas of cancer nursing, research, practice, continuing education, communications and advocacy for better recognition of cancer nursing across Europe which lead to better patient care. In Croatia, there is a national society called Croatian Oncology and Hematology Nursing Society which is a member of EONS since There are cancer nurses in Croatia but not all of them are members of the national society. There is a number of examples that point to the role that EONS plays in oncology nursing, whether in a clinical setting or nursing education. One of them is the EONS Cancer Nursing Framework which is the basis of the cancer care curriculum for nursing students at some medical faculties in Croatia. It would be useful if it was applicable at the national level. The other example is EONS Euro PEPs (Putting Evidence into Practice) guidelines. They are designed to help nurses to improve their use of research evidence in their clinical practice. There are Euro PEPs on Dyspnea, Lymphoedema, Pain, Peripheral neuropathy and Radiodermatitis, in five European languages among which is the Croatian language. Clinical practice guidelines for Management of chemotherapy extravasation, which are the product of cooperation between the European Society for Medical Oncology (ESMO) and EONS, are currently in the process of translating to the Croatian language. Breakthrough cancer pain guidelines and Cancer Therapy - Induced Nausea & Vomiting in Adults are also in the process of translating to Croatian. Croatian national society also applied for financial support to translate Target materials which will be used for the course during the regular national annual meeting. Target course is one of the best possible courses to understand this dynamically growing treatment area and will give nurses the knowledge to improve care and support for patients and their caregivers. More precisely, Target give nurses a better understanding of the characteristics associated with cancer development and how targeted therapies have improved patient outcomes by inhibiting or manipulating these oncogenic characteristics [9]. The Masterclass in Oncology Nursing is specifically designed to meet the educational needs of cancer nurses from Eastern Europe and the Balcan region. The 1st Masterclass was held in Budapest Due to the ongoing COVID-19 pandemic, the EEBR 2020 Masterclass faculty has decided to postpone this event, which was scheduled to take place in November 2020 in Belgrade, to a later date in The Masterclass learning objectives are to understand new developments in cancer disease management, to recognize and describe acute and late adverse effects of cancer and its treatment, to identify complex patient needs across the cancer trajectory and to describe the role of the advanced oncology nurse in leading clinical nursing care [10]. An ideal candidate for this Masterclass would be a nurse with 5-10 years of experience in cancer nursing. And finally, the young cancer nurses should be mentioned. In Croatia, there is a group of young, smart and capable nurses. They are ambitious, eager for knowledge, full of ideas. They are very active in social media and at the same time very active member of the EONS Young Cancer Nurses (YCN) Network. For example, in the EONS' latest project EONS Young Cancer Nurses Nightingale Challenge Webinar Series, YCN Croatia actively participated. CONCLUSION Nursing is a dynamic discipline characterised by the need for quality basic nursing education which is necessary for safe and quality nursing care and the basis for nursing specializations. A life long learning is characteristic of nursing and an obligation for nurses, due to the everyday fast advance in medical sciences and nursing. In order to become as good as possible in the nursing profession, all the available help should be used. Considering cancer nursing, that help is definitely EONS (European Oncology Nursing Society) and all its resources. 36 SESTRINSKA VIZIJA. broj 8. jun 2021.

Pripremili i akreditovali I Međunarodni Kongres, III Simpozijum UUMSZTRS, III Simpozijum sestara u neurologiji, IV redovni Kongres UUMSZTRS.
